HCH state invisible when underwater
When underwater, the bubbles take priority over the 'state descriptor' of hardcore hunger, making it invisible.

That's not a bug. It's an intentional design decision to avoid cluttering the interface.
When underwater, your air supply is the priority status effect, and thus takes precedence over the others.
