Prior to the spawning overhaul you mentioned, spawning rules were entirely vanilla, with the exception that mobs could not spawn on mycelium or mushroom blocks (but could spawn elsewhere in mushroom biomes), which is still true.

Personally I find the large hunger cost to be enough. It encourages the player to be conscious about when they do it, without being too overbearing. Anything more drastic would cause issues with swimming out of things like clay holes anyways, since those also fill with downward flowing water.

Current plan is to update to 1.8, primarily for the technical benefits. The features included in those releases will be kept to the same standards as any BTW feature, but for the most part the biggest features of those versions should be reasonable enough to adapt to fit.

Realism is not an inherent good. Realism is useful as a heuristic so that the player can know what to expect from some mechanics, but not any further than that. Mobs have a chance to despawn when they are more than 32 blocks away, and are guaranteed to despawn at 128 blocks away. Villager sounds were added in MC 1.6. We'll be updating to that eventually (BTW currently runs on 1.6, just a lot of cleanup and bug fixes) so i don't plan on doing anything with that now.
