Tried this program called Blacksmith/Chunkmaster, which is the successor of chunkster, a chunk cleanup tool. Of course it didn't find any bad chunks. Its never easy is it? Continuing to run into the issue in 4.12, i tried running a beta chunk scanner but I cant even be sure if that's working right. Still at a loss here.

Removing the world from RAM did not solve the issue, problem still persists. Also reinstalled minecraft_server.jar from scratch, which has no effect

Unfortunately I've seen this with as little as 4 people on. Running the world outside of RAM for a while to try and slowly eliminate things

Update:

Seems this issue is directly relatd to changing dimensions. Sometimes when users go from Overworld -> Nether or Overworld -> End, this occurs. We actually just caught it in action with jecowa. Really have no idea what the issue is yet tho.

Does anyone know a block auditing tool for vMC? I'm much more familiar with Craftbukkit servers, which is obviously has many administration plugins. Otherwise we just need to catch them red-handed. Either that or we randomly ban people and see if we see a reduction in crime :P
